We want to make sure you are comfortable telling us your story and have permission to share.

Can we use your name publicly if we share your story?
By selecting YES, you are:

• agreeing to allow Texas Healthy Building Blocks state leadership team to share your success story, name, and organization publicly on our social media channels, website, webinars, trainings, and in our newsletters.
• agreeing to allow us to use photos, if uploaded, on our social media channels, website, webinars, trainings, and in our newsletters.
• verifying that you have collected the appropriate consent to share uploaded photos.

If you select NO:

• we will only share your story anonymously (e.g. "a provider", "Ms. Jane") and will share your name, email address, or organization.
